We do our best to give you an estimated co-payment amount prior to your appointment based on information received over the phone from an insurance representative. This information is not a guarantee of payment or approval from the insurance company. Your estimated amount is due at the time of service. We will complete and submit the forms to all insurance companies for benefits. Once insurance processes, if there is a remaining amount due, a statement will be sent to you. Dental insurance is a benefit to help assist you with the cost.  All charges you incur for your dental care are your responsibility regardless of insurance coverage. As your dental provider, our relationship is with you and not the insurance company.

Statements for Owed Balances

Once you receive a statement, please submit the balance as soon as possible. We realize sometimes extreme cases arise that may affect timely payment and if necessary, please contact us directly for assistance in handling this matter or with questions related to billing. Every attempt will be made to accommodate your financial needs.

Our practice accepts most major credit cards. Again, please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
